EMT Test
The National Registry of Emergency Medical Technicians (NREMT) hosts a cognitive examination that candidates must take and pass to become licensed as an EMT.
The cognitive exam is a Computer Adaptive Test (CAT) between 70 and 120 questions long with a 2-hour time limit. Ten of these questions will be “pilot” questions, meaning they are not scored; instead, they are used to gather information for future tests. Candidates do not know which questions are unscored, so it’s important to answer every question to the best of your knowledge.
Except for EMS Operations, all sections have a content distribution of 85% adult and 15% pediatric questions. The exam can be broken down into the following categories:
- Airway, Respiration & Ventilation: 18–22% of exam content
- Cardiology & Resuscitation: 20–24% of exam content
- Trauma: 14–18% of exam content
- Medical/Obstetrics/Gynecology: 27–31% of exam content
- EMS Operations: 10–14% of exam content

NOTE: Passing a psychomotor exam is also a prerequisite for certification, but this skills-based assessment is not administered by the National Registry. Instead, a candidate’s training institution or State EMS Office will offer this exam.
